Possible causes: 

The handset is not set into different groups

Low line voltage

Wiring

 Possible solutions: 

Set the handset into different groups

Double check the wiring and for any loose connections

Check the wiring for any moisture or corrosion.

Make sure that the voltage is between 8-9 volts
